Every 5 years		1) Click on the link to the left and submit a request.
2) Once processed, you will receive an email notification with a subject line “PATCH request completed” and a Control Number.
3) Copy this Control Number to your clipboard and follow the link to https://epatch.pa.gov/RcStatusSearch.
4) Enter the Control Number, your first and last name, and the DATE of the REQUEST.
5) Click on the hyperlinked control number that appears.
6) Click on the hyperlinked “Certification Form” and download this PDF.
7) Use email or a private Google Drive folder to transfer the PDF.
07PA Child Welfare Portal – Child Abuse History Clearance Certificate
(DHS)
Every 5 years		1) Click on the link to the left and submit a request.
2) Once processed, you will receive an email containing a “Please Click Here” link.
3) Once logged in you will see your most recent and past e-Clearance reports.
4) Each report is a represented by a green box which reads “Your application has been
processed. To view the result, click here.”
5) Click on the report to download this PDF.
6) Use email or a private Google Drive folder to transfer the PDF.

09Safeguarding Youth Training (SYT) Certification
Every 2 years		1) This training requires about an hour of your time. The training involves watching
a segmented video and completing a short quiz after each segment.
2) Forward the email with the attached PDF certificate at the conclusion of the training.
10Venture Crew “Trained”
1) This training requires about three (3) hours of your time. The training involves watching
videos and sometimes completing a short quiz after a segment.
2) Follow the link… My Training / Venturing / Learning Plan (Crew Advisor or Crew Committee)
2) No PDF to download. Crew leadership will be notified via unit reports.
